The consequence from COVID-19 is going to have a severe effect on people’s emotional and psychological wellbeing. Amidst social isolation, financial uncertainty, loss of resources and social distancing this is an unprecedented time of stress, anxiety, and fear.

Interesting study. However couple of considerations when considering implementation of telehealth especially for older adults; 

1. Cultural norms need to be addressed. As people always do not trust teleheath

Possible Solution: More educational awareness about the benefits of telehealth and related services

2. Literacy level of the individuals

Possible Solution: Easy to use technology solutions (usability is must)

3. Connectivity and network challenges

Possible Solution: if the network or connectivity is a challenge but can there be provision by telehealth service providers to provide some adhoc connectivity solutions for patients to get enrolled

4. Outcome analysis must

Possible solution: It is must to report not just how many teleconsultations were done but outcomes such as clinical and non-clinical both need to be assessed and documented. 

5. Privacy concerns

Solution: HIPAA’s privacy and security rules require that the information gathered through a telemedicine service is encrypted. We have to be sure that messaging is across a secure connection and that we need to get the permission of patients before we make video calls.

Cost of services and training of older adults is also key to be considered.

Routine care for chronic disease is an ongoing major challenge. Currently most global healthcare resources are focused on coronavirus disease (COVID-19). This resource reallocation could disrupt the continuum of care for patients with chronic diseases.To avoid a rise in non-COVID-19-related morbidity and mortality, including increased depression and anxiety, it is important that patients with chronic diseases continue to receive care in spite of the pandemic. Recent survey response from 202 Health care professionals from 47 countries found that only 14% reported continuing face-to-face care for all consultations, whilst the majority reported being carried out by telephone (45%).  Highlighted use of telemedicine where online video consultations were being used through Zoom, Skype, WhatsApp, Facebook messenger.

“It’s so Important that we all speak up on mental health”- Anne-Marie

I think this COVID-19 Pandemic is a wake-up call for all to build the mental healthcare infrastructure. Many psychological problems such as stress, anxiety, depression and so on emerged during COVID-19 pandemic. Mental health problems are not taken seriously because there has a lot of stigma related to this and also lack of awareness. This thing restricts people from talking about their mental health issues. So, people suffering from mental health issues either commit suicide or harm themselves.

Way Forward

These are some remedies which are helpful to improve mental health infrastructure.

1: Government can make specific preventive strategies at public health level such as effective communication, providing adequate psychological services.

2: Train community health care worker for Ex: ASHA worker, Self Help Groups.

3: Enhance online platforms (telephones help lines, social media, blogs) which are helpful to correctly address stigma and discrimination related to mental health.

4: Develop a scientific committee which should provide appropriate information to attenuate the impact of mental health.

5: Telemedicine should be really implemented especially in areas where mental health services are poorly represented.

6: It is important to make new policies but also very important to effective implementation and evaluation of policies.

This pandemic has produced a lot of uncertainty in the lives of people due to the lack of evidence based information. The misinformation has been one of the key factor causing mental health problems among people of all age groups. The misinformation or lack of correct information related to treatment, preventive measures and self-care is one of the biggest reason effecting people’s wellbeing. Deep penetration of internet, lockdown and quarantine have led to increased screen time which tremendously increased the spread of false information affecting the well being of people. The circulation of false information is not just limited to younger generation infact it is equally affecting older people as the social media usage has increased tremendously among them due to home confined lifestyle. All the false information or no information has led to increased level of anxiety and stress among people. One false news causes elevated stress level among individuals  Some false information which has a deep rooted belief system or which are being practised in a generalized way have become one of the biggest reason for non- effective behaviour change interventions. The misinformation when a person gets COVID+ve or when any family member has been afflicted with the disease leads to sleepless nights about treatment and the progression of the disease affecting the mental health.

On the other hand, various memes or misinformation which circulated actually trivialized the pandemic leading to non-seriousness among people. It also led to non-compliance of preventive measures and hence more spread of the disease, thereby affecting the wellbeing of population at large.

The well-being situation in India is also not good, according to the Happy planet index India achieved only 4.6 points out of 10. The Well-being condition of the Indian slums and other county slums is very poor. According to UN-Habitat, around 863 million people live in slums in the world and according to the Indian Census report around 13.8 million Households-about 64 million people live in city slums. In slums, maximum people live in matchbox size houses and living conditions are very poor. We aren't able to talk about his and her wellbeing situation because we don't know the current scenario of slum people and his and her stress level, feeling, and mental health situation. In the pandemic, the situation of slum livelihood has become very pathetic.

Mental health is indeed one of the most important component to be discussed during times of such uncertainties among every age group but is often neglected by majority. Following handy evidence based techniques to cope with current COVID situation may aid to deal with anxiety and depression. Different scientific Body-mind integration techniques can be followed in daily routine to survive the current situation such as:

1. Mapping our emotions

2. Mindfulness: It is a powerful tool in the treatment of mental health disorders, stress- related conditions etc. It starts with the body and goes into the details of sensations we feel. Our sensory attentional system is one gateway to a richer mind- body connection.

3. Meditation: In traditional meditation, the main focal point for attention training is on the inhalation and exhalation of air. Research into the breath confirms that by being attuned to your breathing, and paying attention to it, naturally slows your breathing. This helps the body relax, resulting in less anxious, depressed and angry.

4. Progressive Muscle relaxations: It is a relaxation therapy which is known to build body intelligence. It teaches us to systematically tense and then release muscles, working on one muscle group at a time. It helps in reducing stress and tension by increasing our focus on the body.

5. Biofeedback: A pioneering technique for building body intelligence is biofeedback. This is the use of scientific and physiological monitoring of the body to effectuate awareness of body states with electrodes. The evidence supporting biofeedback has been fascinating; it can reduce certain disorders such as high blood pressure and migraines. One of the most significant perks of biofeedback is the self-direction that it elicits.

Interesting topic and very relevant to the present  context amidst COVID-19 pandemic. It is not over the worst is yet to come as impact of COVID-19 is seen and now getting deeper and visible and distinct, the economic recession, job cuts, unemployment and burden of other disease and ailments is leading to cycle of deprivation, depression and largely affecting mental health in population. The cases on anxiety, depression and suicidal cases are increasing at alarming rate. Due to COVID-19 not only mental health but healthcare system not able to cope up and  other disease and ailments like  cardiac, cancer, MCH, Diabetes are getting worse affected. People are dyeing as they are not getting timely treatment, care and support as due to COVID-19 bed occupancy and health system are not able to manage critical cases. Cases are being neglected and due to lack of treatment and timely care mortality and morbidity is also at increasing rate. Nevertheless, the stress in family, community and  population is increasing many fold.s I think not only mental well being but physical and social well being is important. COVID & WELL-BEING 

The impact of COVID 19 pandemic is observed in every sector around the world.The outbreak is affecting day to day life of people.So the current challenges brought about by COVID 19 for people all around the world are unparalleled.Due to the home quarantine and lockdowns being put on people around gets easily disturbed.Rumours and misinformation easily spread nowadays which add upon our worries. This causes fear, anxiety, for so many people around the world.As a result the mental health,physical  and social well being is affected severely.So,the word DEPRESSION is being very commonly heard during this period and many easily fall into this trap.

 

But there are many ways in which we can handle the situation around us.

•Always keeps connected with our loved ones by making time to check -in phone, text or  video call.

•As far as physical and mental health is being concerned, we need to have a balance & nutritious diet which produce a healthy body and mind.We can adapt and include exercises,meditation, yoga and breathing exercises such as Pranayama in our practice which can calm down ,relax our minds that reduces stress and anxiety.

•Need to track upon the sleep and wake cycle regularly to check upon our adequate sleep and create it as a DAILY ROUTINE. Comparatively self care is needed more.

 

Ultimately, being compassionate and kind to yourself will lead to a happy life even in tough times. So we need to understand the situation and start to live in our new normal life with ample of happiness. Hope for the best.

It is difficult to maintain a healthy lifestyle when we are in the middle of a crisis like this. The uncertainty and worries related to finances, childcare, elderly parents, and job security disrupt our routines, our lifestyles, and mental health. The uncertainty about the future, the ceaseless news coverage, and a constant social media-driven flood of messages can increase our sense of anxiety. Stress is a normal response to these types of situations. Stress disturbs our sleeping and eating patterns, leads to irritability or emotional outbursts, low motivation, and changes in the use of alcohol or other drugs. Please do not hesitate to seek help if you are experiencing anything like this.* It is also important to maintain a healthy lifestyle and get back into a routine. here are few tips for maintaining good well being

  1. Be kind to yourself. 
  2. Take time out for activities you enjoy.
  3. Take up a relaxation practice
  4. Stick to trustworthy sources
  5. avoiding touching your face (particularly your eyes, nose, and mouth).
  6. staying home as much as possible, even if you don’t feel sick.
  7. avoiding crowds and gatherings of 10 or more people.
  8. avoiding all non-essential shopping and travel.
  9. Make a list of all the possible solutions you can think of. Try not to get too hung up on “perfect” options. Include whatever comes to mind that could help you get by.
    10.Focus on concrete things you can problem-solve or change, rather than circumstances beyond your control

Covid-has adversely affected our lives. This Pandemic has

Affected us physically,  mentally leaving us in disgust!!  

Many people are finding it difficult to cope with mental health  

In the Pandemic.  Negative thoughts,  Negative media , Negative news, Negative environment,  everything Negative has broken us at 

Some point. Words fall short to describe that how this covid-19 has impacted our lives!!!!

Tips to followed to ensure good mental and physical health.

 Exercise- nothing great than this. Even if lazy take a half hour walk.

Eat healthy- Consuming nutritious food can help in achieving good 

Physical and mental health. 

Be spiritually active- Keep faith in universe, god and believe 

Every thing will be back to normal.

Spend time doing your favourite indoor things-  Read, play , dance or spend quality time with your family. 

Work on yourself for yourself.  This is the best time for you to invest in yourself.

Lastly get vaccinated and follow all the safety precaution and motivate others to do the same.
