Patient Generated Health Data (PGHD is gathered from patients or family members. Integration of Electronic Health Records (EHR) facilitates the process of monitoring patients without visiting the hospitals. It will support the remote patient management and timely interventions. It could contribute in closing the healthcare care gaps and aid personalized medicines. Integration of PGHD into EHR is still in infancy stage and development of best practices are required to overcome technical challenges and EHR workflow. More studies on resources and clinical workflows are required leverage the process of integration
